I am considered an out-of-network provider for all PPO plans, including: Anthem Blue Cross, Blue Shield, Blue Card, HealthNet, United Healthcare, Cigna, Optum, Magellan, Value Plus, and many others, depending on your coverage.

This means that you pay my full fee upfront, and your insurance company reimburses you directly. The amount you are reimbursed depends on your specific plan.
